The Mental Health Counselor is a licensed clinician who adheres to the highest standards of evidence based practice, ethics, and professionalism and practices within the appropriate to the scope of licensure. This position provides psychosocial assessments, diagnosis, treatment plans, and consultation to individuals, groups, and families/couples regarding a variety of life issues, behavioral health conditions and substance abuse disorders in a community health system setting. Services are provided in an outpatient setting consisting of diagnosis, treatment and referrals as appropriate. The Licensed Mental Health Counselor works to create an atmosphere that is safe and affirming toward individuals of diverse backgrounds and beliefs, in a culturally competent way. The LMHC will be an integral part of a dynamic interdisciplinary team that includes other therapists, case managers, and primary care physicians.

QUALIFICATIONS:

· Licensed in the State of Texas to practice professional counseling, preferably as an LPC, LPC-A, LCSW, LMFT;

· Bilingual – English/Spanish preferred;

· Excellent oral and written communication skills;

· Outstanding interpersonal skills;

· Experience working with co-morbidities; high-risk populations; Desire to work in a Community Centered Health Home and work to facilitate systems of care with community partners.

ESSENTIAL COMPETENCIES:

· Serves as a core member of the interdisciplinary team with providers and patients to develop a plan of care and monitor quality of care;

· Provide the full range of clinical services: assess client, screen, diagnose, treat and monitor;

· Analyze and evaluate patient data and test findings to diagnose nature and extent of clinical disorder;

· Ability to direct psychotherapeutic treatments and experience referring to medication intervention to treat mental, emotional, or behavioral disorders;

· Gather and maintain patient information consistent with clinic policy;

· Collaborate with the interdisciplinary team and client to contribute to client goals and progress;

· Monitor quality of care and participate in performance improvement and quality assurance activities;

· Complete proper coding and documentation as required;

· Maintain close liaison with the PCP and Case Manager and/or therapist to coordinate transitions of care for patient.
