Licensed Vocational Nurse (LVN)- NAM

  1. Answering phones, scheduling appointments, and data collection for triage RN or medical provider guidance.
  2. Monitors quality of care through direct observation, supervises that appropriate logs are kept including medication inventory, sample medication, oxygen tank pressure, refrigerator/freezer/lab temperatures, immunization inventory, exam room check lists, crash cart log. Counting and logging medication inventories, and vaccine inventories. Ordering clinic supplies, vaccines, and medications as needed.
  3. Monitors referrals, incoming fax documents, notifies patients of lab results from providers written orders and documents attempts and patient acknowledgement in electronic health record appropriately.
  4. Monitors and completes nurse visits with written or standing orders from providers. Performs procedures including but not limited to administering medications, injections, collecting specimens, electrocardiogram, hearing test, vision tests, in-house point of care testing, lab draws, and ear lavage.
  5. Maintains cleanliness and supplies in each patient room, completes room checklists, and notifies appropriate personnel of clinical supply shortages. Disinfects medical equipment and exam room tables after each patient encounter.
  6. Completes medication prior authorizations, and call in refills to a pharmacy under the direct supervision of the medical provider or podiatrist as long as refills per Standing Orders – Prescription Refills. The refill must be documented in the patients chart as a standing order and be patient specific.
  7. Acting as supervisor when provider and registered nurse not on site. Provides counseling to staff as appropriate.
  8. Actively provides willingness to help and maintain professional relationships with coworkers.
  9. Collaboration with Nurse Manager to implement quality initiatives.
  10. Collaboration with Health Center Managers to ensure efficient daily patient flow.

  12. Upholds strict patient confidentiality requirements required by federal and state law.
  13. Perform other duties as assigned, within scope of practice.
  14. Attends all staff meetings and mandatory In Service meetings.
  15. Participates in Compliance, Risk and Safety, and Quality Assurance and Performance improvement initiatives.
  16. Observes and acknowledges all SBCHC policies and procedures, including all emergency and disaster response responsibilities.
  17. Always greets patients, visitors and all SBCHC staff in a courteous and respectful manner; responds to patients with empathy and positive interpersonal skills; consistently handles all requests in a positive manner.

QUALIFICATIONS:

  1. Graduate of an Accredited School of Nursing
  2. Active and current unencumbered Texas license to practice as a LVN
  3. Certified Basic Life Support (BLS) or BLS equivalent
  4. Minimum of one year of nursing experience in Federally Qualified Health Center or similar setting, preferred
  5. Multilingual, English/Spanish is strongly preferred
  6. Available for all shifts/assignments and, when required, able to work evenings and weekends with the ability to travel to all SBCHC clinics.
