Job description
SUMMARY:
Under the direction of the Director of Clinical Quality, the Health Management Information Specialist is responsible for maintaining the security, confidentiality, completeness, and accuracy of the medical records including Release of Information (ROI) requests as well as the management of electronic documents (efaxes) coming into Spring Branch Community Health Center (SBCHC) in accordance with the policies and procedures of the organization including Scope of Work.
QUALIFICATIONS:
- High school diploma or equivalent
- Medical records experience
- Ability to communicate effectively
- Familiar with medical terminology
- Certified as a Medical Records Specialist and/or Release of Information Specialist
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Follow HIPAA policies and laws.
- Verify that all releases of information take place in accordance with Texas State law governing such releases
- Receive, answer and/or respond to phone calls regarding medical records including ROI.
- Ability to communicate well with patients and other professional offices in person as well as on the electronically and on the phone
- Process all incoming and outgoing medical release forms with thorough documentation.
- Scan and append all documents received including processed documents to the patient’s chart in eClinicalWorks (ECW).
- Ability to prioritize workflow and process urgent items timely and accurately.
- Process all medical records subpoenas with supervisor approval.
- Coordinate with supervisor any medical records audit requests/notifications from external agencies and maintain log/appropriate documentation.
- Follow up on retrieval of records requested from outside institutions.
- Actively participate in the yearly review/revision of the medical records protocols as needed.
- Review all incoming faxes via assigned. Efax region/site and assign medical records and other documents to the appropriate folder in ECW.
- Continuously assess for areas of improvement, develop plans and evaluation criteria for such.
- Purge records in accordance the policies and procedures and in accordance with acceptable retention requirements for the State of Texas, licensure and/or accreditation.
- Keep supervisor informed of problems or issues.
- Working knowledge of computer systems, copiers, fax machines, scanning technology. Prior use of Electronic Health Record a plus.
- Responsible for scanning and indexing medical records documents.
- Charge patients and others based on SBCHC’s Medical Records Fee Schedule.
- Participates in SBCHC Compliance, Risk and Safety, Quality Assurance and Performance Improvement (QAPI) activities.
- Other duties as assigned.
Qualifications:
- Graduate of approved high school or GED equivalent
- Medical assistant certificate, Certified Medical Assistant Certificate strongly preferred
- Current Basic Life Support (BLS) or BLS equivalent
- Medical Assistant experience within Federally Qualified Health Center’s or equivalent preferred
- Multi-lingual, English/Spanish required
- Available for all shifts/assignments and, when required, able to work evenings and weekends
